The scheme is formally being ended on 24th February 2022. This means that anyone who was advised to self-isolate before this date is still required to self-isolate based upon the legislation at the time. Eligible applicants will include: a person that has been notified by NHS Test and Trace prior to 24/02/2022 that they have tested positive for COVID-19. a parent or guardian of a child who has been notified by NHS Test and Trace prior to 24/02/2022 that they have tested positive for COVID-19.   an unvaccinated adult contact who has been notified by NHS Test and Trace or identified via the NHS COVID-19 app prior to 24/02/2022. You will need to send in a screen shot of the contact notification to confirm the date that you were notified to isolate. Claimants who are advised to self-isolate before 24th February 2022 still have 42 days in which to submit their application for TTSP from the date of self-isolation so all applications must be received by the Council on or before 6th April 2022. From 19/01/2022 the testing scheme has changed so that you can obtain a positive test result from a Lateral Flow Test rather than having to wait for the results of a PCR test. However in order to be eligible for a TTSP you must have also undertaken a PCR test or Assisted Rapid Lateral Flow Test. You must provide evidence of this when making your claim for the TTSP. You will need to provide details of your 8-digit reference which will have been provided to you by NHS Test and Trace. Without this reference the Council may not be able to verify your details which may result in no payment of TTSP being made.

• People should not normally seek a PCR test if they have already had a positive PCR test within the last 90 days. This means that claimants will be limited to one payment of TTSP within this period. • Persons who are identified as a close contact of someone with the Covid-19 virus will be expected to avail themselves of the current vaccination program so will be limited to only one payment of TTSP, regardless of whether the person then becomes fully vaccinated.

This form is provided to capture the information required by the council to verify your eligibility for payment of the grant.

Before you start please ensure you submit the required evidence as this will delay the process and may mean that you miss out on the payment.

You will need the following information to complete this form:

• Copies of your latest bank accounts account s showing the past 2 months transactions. This includes bank accounts that you hold jointly.

• Proof of self-employment e.g. most recent set of accounts or evidence of self-assessment returns showing your income and expenditure. You will also need to provide trading accounts for the three month period previous to when you self-isolated showing your earnings and business expenses

• Proof of your notification from NHS Test and Trace asking you to self-isolate which will have your unique Test and Trace account ID on it

• Your national insurance number

If you are claiming as a parent or guardian of a child or young person you will not be required to have a Test and Trace account ID for yourself but will also need to provide:

• The name and age of the child who is self-isolating

• That child's education or care setting and year group including an email or phone number for the setting

• And either:

        ◦ the child's eight-digit NHS Test and Trace ID number. This is for parents and guardians of children or young people who have received a notification directly from NHS Test and Trace telling them to self-isolate, AND for parents or guardians of young people aged 16-25 who have additional support needs and who have been notified to self-isolate by the NHS COVID-19 App evidence that the young person has an Education, Health and Care Plan or

Please note: If you are self-isolating yourself then please do not claim as being a parent or guardian of a young person who is self-isolating but make a claim in your own right
